HHS Phase 2 Reporting Update

The Reporting Period for Phase 2 of the Provider Relief Fund is open!

**This is a summary of the reporting guidelines released by the HRSA. To view the official reporting process and all pertinent resources, CLICK HERE.


If you received General or Targeted PRF funds exceeding $10,000 total from July 1, 2020 to December 31, 2020 you are required to report your use of the funds during Reporting Period 2. The deadline to use any funds received was December 21, 2021.


If you received funds during Phase 2, you should have recently received an email from the HHS informing you that the reporting period is open and submissions are to be completed through the PRF Portal.


Overview of the Reporting Process:


Step 1: Register in the PRF Reporting Portal

  • If you received funds in multiple reporting periods, you will only do this once. If you are new to the reporting process, this is required.
  • The registration process takes about 20 minutes and must be completed in 1 session. Read the Registration Manual
  • See below for the required information to register, as listed on the HRSA website:
  • Tax ID Number (TIN) [or other number submitted during the application process (e.g., Social Security Number (SSN), Employer Identification Number (EIN))]
  • Business name of the Reporting Entity (as it appears on IRS Form W-9)
  • Contact information (i.e., name, phone number, email) of the person responsible for submitting the report
  • Address (i.e., street, city, state, five-digit zip code) of the Reporting Entity as it appears on IRS Form W-9)
  • TIN(s) of subsidiaries (if a provider is reporting on behalf of subsidiary(ies) - in a list delimited by commas, e.g.,123456789,987654321,135791357)
  • Payment information (for any one of the PRF payments received)
  • TIN of entity that received the payment
  • Payment amount
  • Mode of payment (check or direct deposit ACH)
  • Check number or ACH settlement date
